Patient's Choice Annual Award Banquet
​
This event recognizes those in the medical field who have provided or contributed to the exemplary services for and the compassionate care of patients.
​
There are several ways in which you can provide support for this effort.
Nominate a Physician or Medical Professional
​
Nominate a physician or medical professional (nurses, logistical support personnel, CNAs, or any employee that work in the medical field) for exemplary service by mailing or emailing the nomination form to swade@jumpsprogram.org.
​
Categories for nominations include the Community Service Award to recognize anyone in the medical field who greatly benefits a community of people or an institution through community services. The Patient's Choice Award is designed to recognize a practicing or retired medical professional that has provided exemplary healthcare service. The Compassionate Service Award will be awarded to a medical professional that has demonstrated compassion in the performance in his or duties.
Financial Gift

By contributing a tax-deductible gift you are contributing to the success of this superb cultural event as well as showing support for our community. The following is a list of sponsorship levels from which to choose:

Prime Co-Sponsor:​The gift of $1500 will purchase a full-page advertisement in the souvenir program and a special reservation for a table of 8 guests. Avendor table for a commercial display of materials to advertise a business, product, or services.
Diamond:​The gift of $1000 will purchase a half page advertisement in the souvenir program and a special reservation for a table for 8 guests.
Platinum:​The gift of $800 will purchase a one quarter page advertisement in the souvenir program and a special reservation for a table of 6 guests.
Gold:​A gift ranging from $300.00 - $600 will purchase a listing as a sponsor in the souvenir program.
Vendor Sponsorship:​A gift of $200 will provide a table for a commercial display of materials to advertise a business, product, or services.
Proceeds from this event will be used to provide outreach missions as it relates to health and wellness to improve the quality of lives throughout the rural areas of the Midlands.
